We are a multi-disciplinary team that includes occupational therapists, speech and language therapists, community learning disability nurses, physiotherapists, sensory practitioners, assistant practitioners and a clinical support team who work across Kent with adults who have a learning disability. We work in four integrated teams across Kent alongside care managers, psychiatrists and psychologists. We work with people who have learning disabilities and health conditions that need specialist care. We provide specialist health advice to people with learning disabilities, their carers, families and the wider health and social care community. We work with our partners to provide care that is centred on the individual with learning disabilities, supporting them, their families and carers by signposting them to and helping them access hospitals and other mainstream services, in line with reasonable adjustments. We also provide specialist direct clinical therapeutic support for people with complex behavioural, physical and mental health support needs.
Menu Close menu. Kent Enablement at Home is managed by Kent County Council and provides short term care and support for adults in their own homes. This includes people who have been discharged from hospital following an emergency, such as a fracture or a stroke.
SCVS supports the identification of needs in the local community and facilitates improvements in service provision to meet those needs. We aim to support organisations and community groups to function more effectively and deliver quality services. Helping people to recognise and develop their ability and potential and organise themselves to respond to problems and needs which they share. Helping organisations with the tools to collect information and assessing it in a method that inform strategic decisions, thus improving the project in the future.
